I have no idea how you could read that Deepwater Horizon article and summarize it as "technological failure." Almost every mistake chronicled in that story is human error, from the bonus-chasing BP execs to the failure of crew members to adequately learn emergency procedures.
All of the technology in the world can't make good things happen when people ignore warnings, fail to follow testing guidelines, or simply make poor decisions in an emergency situation.
Perhaps, to large degree, it was a reliance-on-technology failure.
